Concussion Information and Injury Report Forms

Any suspected head injury should be treated with concern for the well being of the student-athlete.
Forms should be filed with the league coordinator when a student-athlete is being treated with a suspected or diagnosed concussion. Forms are attached.
EAA CONCUSSION POLICY - REQUIRED FOR PARTICIPATION
All student-athletes (and all coaches) participating in any EAA Athletic Program and one of their parents (of student-athletes only), each individually, must view and complete the questions from either of the following concussion courses (only 1 certificate is produced per viewing):

or the CDC “Heads Up Concussion in Youth Sports” – Launch the Course
Choose one course, view video, and complete questions.
Please note: If you choose the NFHS Learn Concussion In Sports Course, you will only be able to produce 1 certificate for 1 person using 1 email (you will need to open an account and log in using 2 different e-mails if the student-athlete and the parent decides to view the NFHS concussion course). The CDC Heads Up in Youth Sports video does not require opening an account and entering an e-mail.
A copy of the student-athlete’s/coach's concussion certificate AND a copy of the parent’s concussion certificate should be uploaded to the student’s EAA registration on the league website. These forms cannot have hand-written signatures.
➜ Read, complete, and upload with the registration the “Parent-Athlete Concussion Information Sheet,” which can be downloaded from the league website. Please note: This form REQUIRES both a printed and signed signature.
Incorrectly completing the form will result in its deletion from the student’s registration, and therefore render the student ineligible to participate.
➜ Complete the “Pre-Participation Head Injury – Concussion Reporting Form,” which can be downloaded from the league website. Please note: This form REQUIRES both a printed and signed signature. Incorrectly completing the form will result in its deletion from the student’s registration, and therefore render the student ineligible to participate.
The EAA considers head injury a serious issue and require that student-athletes/parents:
➜ Report any head injury during the sport season to your Athletic Director/Principal and the EAA by filling out the “Concussion in the Season Report Form,” which can be downloaded from the website (see downloadable forms).
➜ Must complete and file the “Concussion Post Head Injury Clearance Form,” with your school’s Athletic Director/Principal and the EAA PRIOR to returning to participation in any EAA Program once medically declared to have suffered a head injury. This form can be downloaded from the website (see downloadable forms).
